Bridging the Gaps 1 was a research project between 2008 and 2011 funded by Vinnova's Vinnvård.
Bridging the Gaps 1 was a research project (2008-2011) funded by Vinnova's Vinnvård and was run by a research consortium with participants from Jönköping County Council's research and development centre Futurum and Qulturum, the universities of Jönköping, Växjö and Linköping, Uppsala Clinical Centre and Dartmouth Medical School. The project aimed to contribute to substantial improvements in the patient and population health outcomes by improving the system performance and with continual professional development.
Bridging the Gaps 2 was a research project between 2011 and 2013 funded by Vinnova's Vinnvård.
The purpose of this project was to create maximum value and health for patients and the population as a whole, by giving patients the following roles in collaboration with health care:
Co-creators: patients and families are actively involved in the development of health care services.
Co-producers: patients and relatives take the responsibility for parts of their own health care.
Co-evaluators: patients and families actively evaluate the quality of the health care.
